On July 1, 2024, Allevant, developed by Mayo Clinic and Select Medical, is launching a cohort of "Project Swing Bed on the MENDS®," a Critical Access Hospital (CAH) Shared Learning Collaborative. Participating CAHs will have access to Allevant staff, tools and resources to maximize their Swing Bed program and begin to implement MENDS®, a wellness program for healthcare staff and post-acute patients.

Expected Outcomes

 

1) Improved Health Outcomes: Shortened post-acute length of stay, low rate of post-acute patients discharged back to acute care (readmission from post-acute setting), high level of post-acute patients discharged back to independent settings (home or assisted living), increased staff and community focus on wellness behaviors, increased patient wellness, improved long-term health

2) Expanded Capacity for Essential Health Care Services: Increased utilization of CAH swing beds, increase in confidence and capabilities of rural health workforce to provide post-acute care services locally, new focus on wellness strategies for rural staff and post-acute patients, new processes to facilitate rural primary care practices to support continuation of patient-identified wellness strategies for newly discharged rural post-acute patients

3) Increased Financial Sustainability: CAH  - Increased census and revenue, long term reduction in costs associated with increased staff wellness, lower Medicare costs per patient day.

What you will Receive

  • Multi Professional Training and Education – Through training and education, Allevant helps the CAH clinical team become post-acute care experts enhancing the type of patients that can be cared for while also having the ability to obtain CNE credit.  
  • The learning modules include 80 courses which provide a possible 76.2 hours of accredited education.  This includes approximately 40 hours of core CNEs for RN staff, and additional hours of RN enrichment content.  Many courses are also appropriate for nurse assistants, rehab therapy team members, respiratory therapists, etc.  Content areas include Transitional Care Foundations, Respiratory, Cardiac, Neurology, Complex Systems, Tracheostomy, Ventilator, Diabetes, Hyperlipidemia, Hypertension, Managing Patient Family Relations, Medication Safety, Mental Health, Obesity, Prevention of Skin Breakdown, Sepsis, Traumatic Brain Injury, and Trauma Informed Care to name a few broad areas.
  • Available resource documents include 185 user guides, templates, graphics, references, and best practice guidelines on topics including; care coordination, clinical care, education support, human resource support, leadership resources, quality, outreach and marketing, and database and user access.
  • Project Management and Collaboration with Peers – Allevant will work collaboratively to create a project timeline based on prior successful implementations and current situations with measurable goals.
  • Licensing and use of the brand – Utilize the name “Allevant developed by Mayo Clinic and Select Medical” and any outcomes data or story points for referral education purposes to help tell the story publicly.
  • Clinical and Workflow Process Reengineering – With implementation led by Dr. Lindsay M.D. M.M.M, Allevant uses best practices to create a care environment that leads to high patient and employee satisfaction and improved patient outcomes. 
  • Referral Education Development and Support – Allevant will assist with referral partner and community program education efforts by helping to create compelling education material.  Allevant will work with the CAH team and key referral partners (Administrators, Doctors, Case Managers etc.) to share data and help tell the story of why Transitional Care will help their organization and most importantly their patients. 
  • Quality Database Management Utilizing Core Key Indicators – Easy access to a HIPAA compliant online database and leadership dashboard that can be used for benchmarking with peers and marketing purposes. Measuring key outcomes is critical in demonstrating the effectiveness of the program.
  • Continued Program Development and Support – Transitional Care becomes a “Golden Moment” for helping Transitional Care patients and staff focus on wellness and prevention of burnout using a program called MENDS®.

With a strong focus on quality, outcomes, and growth, our clients saw an average growth in swing bed days of 55% by the second year.  64% of patients were categorized as complex medical or complex surgical which is a growing patient population, and only 7% of Transitional Care patients were discharged directly back to acute care during a program stay of less than 30 days.  76% of patients were discharged from Transitional Care back to independence.  The average patient rating was 4.83 out of 5 when asked if patients would “recommend the program to others”.
